Application and Information Request

This Data Subject Application Form ("Form") has been prepared to enable data subjects to exercise their rights under Article 11 of the Law on the Protection of Personal Data No. 6698 ("KVKK").

Within the scope of the KVKK, you may apply to the data controller and exercise the following rights regarding yourself:

• To learn whether your personal data is being processed,
• To request information if your personal data has been processed,
• To learn the purpose of processing your personal data and whether such data is being used in accordance with its purpose,
• To learn the third parties to whom your personal data has been transferred, whether domestically or abroad,
• To request the correction of your personal data if it has been processed incompletely or inaccurately,
• To request the deletion, destruction, or anonymization of your personal data within the framework of the conditions stipulated under Article 7 of the KVKK,
• To request that correction, deletion, destruction, or anonymization operations be notified to third parties to whom your personal data has been transferred,
• To object to the occurrence of a result against you by means of the analysis of your processed personal data exclusively through automated systems,
• To request compensation for damages in the event that you suffer damage due to the unlawful processing of your personal data.

Your applications may be submitted to us pursuant to Article 13 of the KVKK. Your application will be evaluated and concluded as soon as possible according to its nature and, in any case, within the period stipulated under the Law.

In order to ensure that your personal data is kept accurate and up to date, it is important that you notify us of any changes to your information.

In order for your application to be evaluated in a proper and secure manner, necessary information and documents may be requested for the verification of your identity. Applications submitted by unauthorized persons or applications for which sufficient verification cannot be provided may not be taken into consideration.
