This Personal Data Subject Application Form (“Form”) has been created to be used by data subjects in their applications to exercise their rights specified in the Personal Data Protection Law No. 6698.

Accordingly, you have the following rights:

• Learning the purpose of processing your personal data and whether it is used in accordance with the purpose;
• Learning the third parties at home and abroad which your personal data is shared with;
• Requesting the correction of your personal data when it has been processed wrong and incomplete;
• Requesting the deletion or destruction of your personal data in case the reasons requiring the processing of personal data disappear; and
• Requesting notification of the transactions made pursuant to subparagraphs e) and (f) to the third parties to whom your personal data has been transferred.

It is important that the personal data processed by our company is accurate and up-to-date.
